硬脑膜动静脉瘘(DAVF)是指硬脑膜内的动脉与静脉之间存在异常交通。DAVF的病因尚未完全明确,可能与先天性血管发育不良、后天性静脉窦闭塞、颅脑外伤、感染、外科手术等因素有关。影像学检查是诊断DAVF的关键,其中数字减影血管造影(DSA)是诊断的金标准,CT血管成像(CTA)和磁共振血管成像(MRA)可作为辅助筛查手段。治疗方式包括保守治疗、血管内介入治疗、显微外科手术、立体定向放射治疗及复合手术。血管内介入治疗是目前的主要治疗手段。综述了DAVF病因、临床表现及复杂DAVF的辅助检查、治疗方法等,影像融合与多影像技术结合可简化诊断、优化治疗。
Abstract:Dural arteriovenous fistula(DAVF) refers to an abnormal communication between arteries and veins within the dura mater. The etiology of DAVF remains incompletely understood and may be associated with congenital vascular malformations,acquired venous sinus occlusion,craniocerebral trauma,infection,surgical procedures,and other factors. Imaging examinations are crucial for the diagnosis of DAVF,among which digital subtraction angiography(DSA) is the gold standard,while computed tomography angiography(CTA) and magnetic resonance angiography(MRA) can serve as adjunctive screening tools. Treatment options include conservative treatment,endovascular therapy,microsurgery, stereotactic radiotherapy, and hybrid surgery. Endovascular therapy is currently the main treatment modality. This article reviews the etiology, clinical manifestations,auxiliary examinations,and treatment methods of DAVF,especially those for complex DAVF. In addition,the combination of image fusion and multimodal imaging techniques can simplify diagnosis and optimize treatment.
